(Hospitals often bill an amount less than the full charge due to a contracted arrangement between the hospital and the insurance company. This discount may be substantial.)

You will need to clarify which health care providers are in your network, as charges may increase with out-of-network providers.

Determine your remaining Deductible. You will be responsible to pay at least this amount.
Alternately, you will be responsible for your Copay amount.

The Out-of-Pocket Maximum is a general guide to what your total payment might be, but verify all of these figures with the insurer you have listed.
